The last connection made should be on the dead car, the negative cable, at a good ground away from the battery (usually the engine). This last connection can be on the "live" car if no suitable ground is available on the dead car. This should also the first cable removed after jumping. This way the sparks generated when the cable is installed and removed will be away from the hydrogen (i.e. expolsive) gas being given off of the batteries. This will lessen the chance of a battery expolsion. You should also wear goggles when jumping a car, I keep a set with my jumper cables (so in the event of an explosion you will only be burned and not blinded).
