First I would recommend reading the instructions, but the procedure is basically as follows.

Lift and secure the car.
Remove the wheel.
Remove the upper bolt and loosely replace with new bolt using the same bolt orientation.
Loosen the lower bolt.
Push the brake rotor at the top to increase the negative camber.
Tighten and torque the upper bolt.
Tighten and torque the lower bolt.
Replace and torque the wheel lug nuts.
Lower the vehicle and schedule an alignment.

Alternatively, just bring the camber bolts to your scheduled alignment and provide the amount of camber (and other alignment specifications) that you want.

I typically have worked with Specialty Products alignment aftermarket products, but if I understand correctly, the "washer with handle" will be next to the head of the bolt.

You should realize that the position of the handle will not necessarily be the same for the right and left side. It is one way to increase or decrease camber when both the upper and lower bolts are loose.

Ingalls? I'm running SPC but they look the same when I checked their website. Even the part number is the same.

Here is the instructions for the SPC.. it says tabs point away from wheel for negative camber.

I have "Eibach" camber bolts, they are really SPC bolts in an Eibach box

Really I think they're all SPC bolts.
